For providers who want better patients

Vital makes software that guides patients through hospital, emergency, and urgent care visits, and supports them after discharge. We are HIPAA-compliant, HITRUST CSF certified, and SOC 2 audited. We partner with health systems across the US, including Emory Healthcare, Dignity Health, and Ascension, among others.

Your practice is already listed on Vital Care Finder, built from the national provider registry. Verifying confirms what we hold is right: the number patients call, the specialties you cover, the insurance payers and plans you support, and whether you are accepting new patients. It also adds a Verified tag and a profile photo to the listing.

It is free, there is no contract, and there is nothing to install. A practice that wants no new patients still has a reason to verify: it is how the calls you do get are ones you can actually help.

Neither is a bought lead list, and we do not sell your details.

We source fax numbers from public healthcare directories, including the national provider registry. If the number on file is incorrect, or you’d like it updated, contact support with your name, NPI, and the correct fax number.

Four ways. Three of them start with a discharge note from a partner health system, and the fourth is someone choosing you themselves. Vital works with 3 of the top 6 health systems in the US, and runs the note-based outreach as the Follow-Up Outreach Initiative, which is the name printed on the fax.

• The note names your practice. A patient is discharged and told to follow up with you by name, so we match them to you. These are always free, however many arrive.
• The note names a specialty. The patient is told to follow up with a cardiologist, or any other specialty, and we recommend a practice that fits. Providers at the health system come first.
• The practice on the note cannot take them. Where the named practice, or the health system’s own provider, is not accepting, we recommend someone who is.
• The patient chooses you. Someone looking for care picks your practice, on the web or through an AI assistant such as ChatGPT or Claude. This is the only way patients arrive through our MCP apps and the Vital Care Finder website.

Every one of these is a recommendation, and a follow-up patient asked for the outreach themselves through the health system’s patient app. You see who is coming and why they are a fit, and you decide whether to accept.

A follow-up fax includes a brief visit summary. It is not the full record. For that, contact the hospital named on the fax directly.

A Vital Care Finder patient has no hospital visit behind them, so there is no summary. You get the reason they are looking for care in their own words, their insurance plan, and their phone number.

Either way, we recommend confirming the reason for the visit, any clinical details, and insurance information directly with the patient during your callback.

Call the phone number printed on the fax and enter the access code when prompted. You’ll be connected directly to the patient.

On Grow Your Practice you can also take inquiries by online booking link, so a patient books a time with you instead of waiting for a callback.

A few things to know:

• Your phone number is not shared with the patient. Outbound caller ID is masked on both sides.
• The call is recorded so we can confirm whether an appointment was booked. You do not need to report back to us.
• Recording disclosure is handled by Vital. You do not need to notify the patient yourself.
• The access code is valid for 15 days from the date on the fax.

Turn them down. You see who is coming and why they are a fit before you accept, and declining costs you nothing.

If you are full, set your listing to not accepting new patients. It stays accurate, and you receive nothing. That is separate from opting out of faxes altogether, which stops us contacting you at all.

Claiming and verifying your listing is free, and stays free. So is receiving a fax and calling a patient back.

If a doctor at a health system recommended your practice by name, that patient is always free, however many arrive. When we are the ones recommending you, that is the patient that counts against your allowance.

A free listing includes three of those a year, taken by phone. Grow Your Practice lifts the cap for $25 a month, with unlimited inquiries by phone, fax or online booking link. It is free until January 1, 2027, a month that brings you no inquiries rolls over at $0, and you can cancel at any time.

You pay nothing for a patient you turn down. On Grow Your Practice, a no-show is credited when you tell us.

We will tell you before anything about your listing starts costing money.

No. There is no contract to sign, no legal review, and nothing to install. Verifying takes about ten minutes. You need your NPI, a work email, and whether you are taking new patients.

You can change what your listing says, or stop receiving patients, at any time.
