Your practice is already listed on Vital Care Finder using data from the national provider registry. Verifying your listing lets you confirm your details are correct and update anything that isn't.

Patients choose who to call based on your specialties, accepted insurance, and whether you're taking new patients. Keeping these accurate means the calls you get are ones you can help with.

Verified listings show a Verified badge and can add a profile photo.

No. Every patient we send you has asked for care:

• Follow-up patients were discharged from a partner health system and asked us for help arranging their next appointment.
• Vital Care Finder patients searched our directory and chose your practice.

These aren't bought leads, and we never sell your details.

We source fax numbers from public healthcare directories, including the national provider registry. If the number on file is incorrect, or you’d like it updated, contact support with your name, NPI, and the correct fax number.

Patients reach you in two ways:

• They found you on the Vital Care Finder website, or through our apps in ChatGPT and Claude.
• We recommended you through our Follow-Up Outreach Initiative, which we run with three of the top six health systems in the US. These patients asked for help through their health system's app after being discharged.

For follow-up patients, we'll recommend you when:

• Their discharge note names your practice. A doctor at the health system told them to follow up with you by name. These patients are always free.
• Their discharge note names your specialty. For example, the note might say to see a cardiologist, a dermatologist, or a physical therapist. We'll recommend a practice that fits, starting with the health system's own providers.
• The practice on their discharge note can't take them. If the named practice or health system provider isn't taking new patients, we'll recommend one that is.

You see who the patient is and why they're a good fit before you decide whether to accept.

What you receive depends on how the patient found you:

• Follow-up patients come with a brief visit summary. If you need their full record, contact the hospital they were discharged from.
• Vital Care Finder patients haven't had a hospital visit, so there's no summary. You'll get the reason they're looking for care in their own words, their insurance plan, and their phone number.

Either way, we recommend confirming the reason for the visit, any clinical details, and insurance information with the patient when you call them back.

Call the number on the fax and enter the access code when prompted. We'll connect you directly to the patient.

On the Grow Your Practice plan, you can also add your online booking link, so patients can book directly through the scheduling system you already use.

A few things to know:

• Neither you nor the patient see each other's phone number.
• Calls are recorded so we can confirm whether an appointment was booked. We let the patient know, and you don't need to report back to us.
• Recording disclosure is handled by Vital. You do not need to notify the patient yourself.
• The access code lasts 15 days from the date on the fax.

Turn them down. You see who is coming and why they are a fit before you accept, and declining costs you nothing.

If you are full, set your listing to not accepting new patients. It stays accurate, and you receive nothing. That is separate from opting out of faxes altogether, which stops us contacting you at all.

Claiming and verifying your listing is free, and stays free. So is receiving a fax and calling a patient back.

If a doctor at a health system recommended your practice by name, that patient is always free, however many arrive. When we are the ones recommending you, that is the patient that counts against your allowance.

A free listing includes three of those a year, taken by phone. Grow Your Practice lifts the cap for $25 a month, with unlimited inquiries by phone, fax or online booking link. It is free until January 1, 2027, a month that brings you no inquiries rolls over at $0, and you can cancel at any time.

You pay nothing for a patient you turn down. On Grow Your Practice, a no-show is credited when you tell us.

We will tell you before anything about your listing starts costing money.

No. There is no contract to sign, no legal review, and nothing to install. Verifying takes about ten minutes. You need your NPI, a work email, and whether you are taking new patients.

You can change what your listing says, or stop receiving patients, at any time.
